HFLW Brand is Hiring a Wellness Coordinator Near South Boston, MA

The Home for Little Wanderers' Harrington House is looking for a Wellness Coordinator to care for children in their Dorchester location. In the group home, children or adolescents, ranging in age from 8-15 are in transition back to their families or going on to independent living, but need support in order to make a successful transition back to their communities.

The program provides residential treatment by offering a safe, structured home-like, living environment. The children participate fully in the community through school attendance and extracurricular activities. Additionally, the programs offer clinical services, health services, psychiatric services, educational assistance, milieu therapy, and case management services.

Principle Responsibilities:

Under the direction of supervisor and Nurse Monitor, administer medication to clients, perform duties consistent with Wellness Office responsibilities. The Wellness Coordinator is expected to develop an understanding of the program’s mission, treatment philosophy and policies and procedures and to implement this understanding in their direct care and supervision of clients.

How You Will Be Making a Difference

  • Administer medications to group home clients per MAP standards; complete incident reports for medication refusal
  • Transcribe med orders.
  • Set up monthly med sheets.
  • Act as liaison for pharmacy orders submitted by the Nurse Monitor (i.e. calling the pharmacy to make sure that we have all of the prescriptions for admissions, and reordering when running low).
  • Sign in meds that are delivered by the pharmacy.
  • Assist families in monitoring needs, scheduling and attending all necessary health appointments for their children such as routine care, assessments, labs, EKGs, etc.
  • Prepare consult sheets per order of MD and at the direction of the Nurse Monitor.
  • Maintain/order supplies: including over the counter meds, program wellness supplies such as band aids, sanitary products, etc.
  • Maintain program First Aid Kits.
  • Maintain health related records of group home clients as needed including health/wellness related activities, incident reporting, periodic vital signs and weights, filing external provider reports, consults, etc.
  • Attend residential treatment teams.
  • Prepare and assist in group home client admissions.
  • Assist families/caregivers in providing the program with copies of required documentation such as annual physicals, dental exams, immunization records, TB records, etc.
  • Review and document client health related information in a communications log.
  • Update and distribute communications about client allergies.
  • Verify invoices related to health/wellness purchases such as pharmacy orders, over the counter supplies, etc.
  • Liaison with school nurses around medication plans and coordination of health related needs.
  • Pack medications for client leaves of absences, or coordinate with pharmacy for same.
  • Liaison with community based health providers such as medical, dental, vision, etc and insurance companies.
  • Perform special projects and other duties as assigned.

  • Become familiar with programs routines/ structure. Develop a general understanding of the population of clients we work with.
  • Foster healthy, appropriate relationships with our clients through daily interactions with them.
  • Develop an understanding of Behavior Support System).
  • Provide First Aid appropriately as needed.
  • May plan and participate in activities with students on an individual and small group (less than 6) level in accordance with treatment plans and goals. Teach age appropriate life skills and social skills through modeling, counseling, and other techniques. 
  • Support Direct Care staff in maintaining a safe and supportive environment. Manage living, dining, educational, recreational areas to provide a clean, organized environment for students. Insure adequate supervision of students through coordination of care with other staff.
  • Provide group and individual behavior support and intervention as trained, including the use of TCI appropriate physical restraint as necessary. Coordinate medical and emergency situations with the supervisor.
  • Attend trainings
  • Participate in supervision meetings with supervisor.

Education and Experience:

High school diploma or equivalent. 1 year child care experience in a residential setting with similar population of children. MAP certification required.

Physical Requirements:

Continuous walking, standing, climbing, lifting of objects for 6 – 7 hour shifts. Ability to effectively restrain using proper TCI techniques when needed. Ability to read and write. Must be flexible, may have to work extended shifts. May be exposed to kicking, punching, spitting children.
